California Simplified Financial Statement
California Simplified Financial Statement forms to be used in California courts for a dissolution action.
Under specified circumstances, spouses may use a simplified financial statement (Judicial Council Form FL-155) in place of the Income and Expense Declaration and its accompanying pages (Judicial Council Form FL-150).
You are eligible to use a California Simplified Financial Statement only if
- Neither you or your spouse is requesting spousal support or a modification of spousal support;
- Neither you or your spouse is requesting an award of attorney fees;
- The filing spouse receives no income from any source other than welfare, salary or wages, disability, unemployment, interest, worker’s compensation, social security, or retirement; and,
- The filing spouse is not self-employed.
When you a California Simplified Financial Statement form, you must attach copies of your pay stubs for the last two months, and pay stubs received with any other income, to the form and must bring to the hearing a copy of your latest federal income tax return. Copies of the pay stubs, other payment stubs, and tax return must be attached to the copy of the form which is to be served on the other spouse.
